If your child will be participating in the school vaccination clinic, please return the attached completed form to the nurses by May 5, 2026. If you need a paper copy of the form your student can pick one up in the nurse's office. Dear Parent / Guardian, If you would like to participate in the Health Heroes School Located Vaccination Clinic held on May 7, 2026, at your child's school - complete in full and sign the consent form attached. There is no out-of-pocket charge to parents for this service. If your child has Medicaid, AllKids, or private insurance, HNH Immunizations will bill the insurance company for the vaccine. If your child is uninsured, the vaccine will be given free of charge. If your child is covered by PEEHIP, we cannot provide vaccinations for your child. We apologize for this inconvenience- please contact PEEHIP at 1-877-517-0020. Please see www.immunize.org or www.cdc.gov for the current Vaccine Information Statement or more information regarding each vaccine recommended by the CDC Advisory Committee on Immunization Practices (ACIP). Current 8th graders playing 9th grade football next fall: a parent meeting will be held on Monday May 11th. We will meet in the auditorium at 6 pm.We will be discussing our upcoming summer schedule or other important details. Physicals will be held on May 5th at AHS after school. There will be 2 buses in the front of the bus line to be transported to AHS. Parents will pick you up at AHS. Please see Coach Yarbrough with any questions!

Over the summer, our team will clean and update the devices to ensure all is ready to go for the 2026-27 school year!CHARGER NOTE: Students will not turn in their iPad charger. They are expected to keep this charger for the next three years. Next year, only students who are new to ACS will receive a charger with their iPad.If your student turns in a device with damage to the screen or case, a fee will be placed on your PowerSchool account.
