The preschool classes will be traveling to the Children's Discovery Museum in Normal, Illinois on Friday, April 17, 2026. Please complete this form by Friday, March 20 to let us know if you give permission for your student to participate. If you do not complete this form, your student will not be able to attend the field trip. There will NOT be PreK class on this day if you choose not to attend the field trip. PreK field trips require an adult to accompany the student. Due to limited space on the buses, we will be sitting three to a seat. No other children are permitted to attend. Everyone will be transported by bus; however, if you want to drive your own personal vehicle with your child, this will be permitted. Please understand that you will NOT be reimbursed mileage or parking fees if driving on your own. The bus will depart promptly from Northwest Elementary School at 8:30 a.m. and return at approximately 2:30 p.m. Please arrive to the school by 8:15 a.m. so we can be sure to leave on time. There is NO cost for your child and his/her accompanying adult to participate in this field trip. All admission fees are paid for by the district and the prek grant. Please bring a lunch for yourself and your child. Please do not bring any glass or breakable containers. If you have any further questions, please contact your student's teacher. Si no completa este formulario, su hijo/a no podrá asistir a la excursión. NO habrá clases de PreK ese día si decide no asistir. Las excursiones de PreK requieren que un adulto acompañe al estudiante. Debido al espacio limitado en los autobuses, se sentarán tres personas por asiento. No se permite la asistencia de otros niños. Todos serán transportados en autobús; sin embargo, si desea conducir su propio vehículo con su hijo/a, se le permitirá. Tenga en cuenta que NO se le reembolsará el kilometraje ni las tarifas de estacionamiento si conduce solo/a. El autobús saldrá puntualmente de la Escuela Primaria Northwest a las 8:30 a. m. y regresará aproximadamente a las 2:30 p. m. Por favor, llegue a la escuela antes de las 8:15 a. m. para asegurarnos de salir a tiempo. La participación en esta excursión es gratuita para su hijo/a y su acompañante.
