Any student in grades six through eight who would like to participate in middle school athletics must submit the sports physical forms prior to the deadline. If your child already has a current sports physical on file in the health office that will not expire before tryouts, then only a Mini Sports Package is required. This includes: a health update form completed by a parent or guardian and a permission slip. If your child does not have a current sports physical or if the physical will expire before tryouts, then a Full Sports Package (which includes a new physical) must be submitted. Upcoming Due Dates Spring Season (Track): 4/6/26 NOTE: We do not accept faxed or copied forms. Only originals are accepted.
