Registration for the campaign will be taken at the Zero Session. To get more people playing, we will ask that if you play in one campaign, you take the next one off. This assistance is provided by grant funding through SAMHSA to help address social determinants of health. Social determinants of health are the aspects of living that affect people’s health, well-being, and quality of life, like access to transportation, food, childcare, education services, mental and physical health providers, social connections, etc. Care navigation is provided through one-on-one meetings on a walk-in basis. We understand that everyone's needs are unique, and we will determine, collaboratively, what barriers and needs exist and work together to develop a personalized plan. Walk-ins only, no appointment needed.
